PDCA (Plan–Do–Check–Act) is the basic cycle of continuous improvement. The steps of a Kaizen record follow it.

  • Plan: define the problem, measure the current state, write the target.
  • Do: try the change on a small scale.
  • Check: measure the result, compare with the target.
  • Act: standardise if it worked, re-plan if it didn't.

It isn't done until the loop closes

Many improvements stop at “do”: the change is made but not measured. Because measurement and sustainment follow-up are separate steps in the record, a half-finished cycle is visible.
